Do the lines that run down from the corners of your mouth bother you? Those marionette lines quickly give your face a tired or somewhat sad expression, even if you don鈥檛 feel that way at all. The big question is usually: should you tackle them with Botox or filler? On this page, you鈥檒l learn what exactly causes marionette lines, why Botox and fillers each do something different, and which choice (or combination) suits your face. No sales pitch, just an honest explanation so you can start a consultation well-prepared.

Marionette lines are the vertical lines that run from the corners of your mouth down towards your jawline. They鈥檙e named after ventriloquist dummies, where the mouth moves with hinged lines. In some people they are fine lines, in others they are real grooves that visibly pull the mouth corners downward.
They develop through a combination of factors. Over the years, you lose volume and firmness in the lower half of your face, the skin becomes more pliable and gravity does the rest. Additionally, a specific muscle, the depressor anguli oris, actively pulls your mouth corners down. These two causes, volume loss and muscle action, are why Botox and fillers each play a different role here.
Filler adds volume. For marionette lines, a filler restores lost volume and support around the mouth corner and the groove itself, so the line becomes less deep and the mouth corner lifts along with it. Because loss of structure is the main cause, filler is often the foundation of the treatment.
Botox relaxes. A small, precise amount in the muscle that pulls your mouth corner down makes it pull less hard, so the mouth corner can subtly lift. Botox therefore does not add volume, but removes the downward pull. In many faces, the combination of both gives the most natural and balanced result.
Which approach is right for you depends on what mainly causes the line: if it鈥檚 depth and volume loss, the emphasis is on filler. If it鈥檚 mainly an active downward pull of the mouth corners, then Botox can do a lot. During a consultation, we look at that together.
If you have clear grooves and your mouth corners sag due to volume loss, filler is usually the logical first step. If your lines are fine and your mouth mainly points downward because of muscle action, then Botox alone can make a difference. For pronounced marionette lines, we often work with a thoughtful combination.
Sometimes an injectable is not the best solution. In the case of severe skin laxity or excess tissue, adding volume can even emphasize the heaviness, and another path is more honest. Also with an active infection or inflammation in the area, during pregnancy and breastfeeding, or with certain conditions and medications, we do not treat or not immediately.
We will tell you honestly when a treatment will add little or when your expectations are not realistic. Rather an honest no than a result you won鈥檛 be happy with.
Everything starts with a calm conversation. We look at your entire lower face, not just the line itself, and discuss what bothers you and what you want to achieve. Based on that, we propose a plan: filler, Botox or a combination, with an explanation of why each time.
The treatment itself is short. We apply filler precisely with fine needles or a microcannula, often with a numbing component in the product or local anesthesia for your comfort. Botox goes into the muscle with a thin needle and is done in a few minutes. Afterward, you can usually just continue your day.
We work step by step and rather cautiously. Preferably a natural, subtle result than too much at once, because adjustments can always be made, but reversing is more difficult.
With filler, you see the effect almost immediately: the groove becomes softer and your mouth corners look less downward. In the first days there can be some swelling, so the true final result is seen after about one to two weeks, when everything has settled.
Botox needs time to take effect. You usually notice the relaxation of the mouth corner within a few hours to 3 days, with the full effect after about two weeks. That鈥檚 why we sometimes smartly plan a combination treatment in phases.
How long it lasts varies per person and per product. Filler in this area often lasts several months to over a year, Botox generally a few months. Together we find a rhythm that suits you and your face, without over-treating.
Both treatments are generally well tolerated, but completely without risk does not exist. After filler you may temporarily have some swelling, redness or a bruise around the mouth corner. With Botox there is a small chance of temporary asymmetry or a feeling of less control around the mouth if the dosage were too generous, which is why we are very cautious here.
With filler there is a rare but serious risk if the product enters a blood vessel. That is why anatomical knowledge and safe technique are crucial, and why you best choose a doctor who knows this area well and knows how to act if needed.
Are you unsure if injectables are for you? Then feel free to book a consultation. We look at your face honestly, explain the options and decide together whether, and how, we tackle your marionette lines.
We deliberately don鈥檛 mention a fixed price here, because it depends on what you need: only filler, only Botox or a combination, and how much product is responsible for that. A subtle refinement does not cost the same as a pronounced correction.
During the consultation you receive an honest estimate, tailored to your face and what is realistically achievable. That way you know what to expect beforehand, with no surprises afterward.

With marionette lines, it is about more than just the right injectable; it is an interplay of muscle activity, volume and skin structure. That is why it makes sense to choose an approach that sees the whole picture.
I only combine botox and filler when that respects your facial features, never at the expense of your own expression.
My surgical background helps me understand the deeper cause of your marionette lines, not just the superficial fold.
We do not think in terms of a single correction, but of a long-term plan that firms your skin with regenerative techniques such as Sculptra or skinboosters.
